1. Introduction
Desha AI ("Desha," "we," "our," or "us") operates the MedNote mobile and web application, a healthcare technology platform designed to support personal health management, prescription tracking, and medication adherence. The platform includes an AI voice assistant that telephones you and holds spoken conversations about your medication. We are committed to protecting your privacy and handling your health information with the utmost care and transparency.
This Privacy Policy describes what data we collect, why we collect it, how we use and protect it, and your rights regarding your personal and health information. It applies to all users of MedNote across iOS (Apple App Store), Android (Google Play Store), and our web platform.
By creating an account or using MedNote, you agree to this Privacy Policy. If you do not agree, please do not use our services.

3. Information We Collect
3.1 Account and Identity Information
- Full name, date of birth, and gender
- Email address and mobile phone number
- Profile photograph (optional)
- Family member profiles added by the account holder
3.2 Health and Medical Information
This is sensitive data. We collect it only to provide core health features and only with your explicit consent.
- Prescription documents and medication details (uploaded by you)
- Medical history and chronic condition records (self-reported)
- Medication schedules and adherence logs
- Health Responsibility Score (HRS) data points
- Vitals and health readings you choose to log (blood pressure, glucose, etc.)
- Doctor consultations and clinical notes (if connected to DocNote)
- Healthcare provider information
3.3 Usage and Technical Information
- App interactions, features accessed, and session duration
- Device type, operating system version, and unique device identifiers
- IP address and approximate location (country/city level)
- Push notification interaction data
- Crash reports and app performance diagnostics
3.4 Device Permissions We Request
- Camera / Photo Library: To upload prescription images and profile photos
- Microphone: To carry your side of an in-app AI voice call. Your speech is streamed live for transcription while the call is connected. See Section 6
- Notifications: To send medication reminders, health alerts, and incoming in-app call alerts
- Biometrics (Face ID / Fingerprint): To secure your account login (optional)
You can revoke any permission at any time through your device settings. Revoking certain permissions may limit app functionality.
3.5 Voice Call and AI Conversation Data
Desha places and receives AI voice check-in calls, and hosts in-app AI conversations. This produces its own category of data. Section 6 describes it in full.
- Your speech during a call, streamed live to a speech-to-text service while the call is connected
- A written, word-for-word transcript of the call, both your turns and the assistant's
- An AI-generated written summary of the call and its outcome
- An audio recording of the assistant's spoken side of a telephone call. We do not store an audio recording of your own voice
- Facts the AI extracts from the conversation (for example a symptom you mentioned, an allergy, a medication you confirmed, a preferred call time) and keeps so later calls have context
- Call metadata: the phone number called and called from, date, time, duration, language, direction, the telephony carrier used, and an urgency classification
The same handling applies to in-app AI calls and to AI chat sessions with the Health Companion.
3.6 Payment Information
The MedNote iOS app is completely free and does not offer any In-App Purchases. Protect Plus subscriptions are purchased exclusively through the Desha AI website at desha.ai.
Web-based subscription payments are processed by our third-party payment processor. We do not store your full payment card details on our servers. Billing records are retained for legal and tax compliance purposes.
4. How We Use Your Information
Providing and personalising MedNote
To deliver medication reminders, track adherence, compute your HRS, and display your health timeline.
Prescription Intelligence Processing
AI-assisted extraction of medication names, dosages, and schedules from uploaded prescription images. Processing occurs on secure servers and extracted data is linked to your profile.
AI Voice Check-In Calls
To call you about a due or missed dose, answer your medication questions, and record what you tell us so your care record and future calls stay current. Calls are transcribed and stored. See Section 6.
Family Health Management
To enable the account holder to manage health profiles for family members (including minors) under a single subscription.
Communications and Notifications
Medication reminders, health alerts, appointment notifications, and service updates. Marketing communications only with your explicit consent.
Safety, Security, and Legal Compliance
Fraud prevention, platform security, and compliance with applicable healthcare data laws including GDPR, DPDP Act (India), HIPAA (where applicable), and CCPA.
Service Improvement and Research
Aggregated and anonymised data may be used to improve platform features. We do not use identifiable health data for research without separate, explicit consent.
5. Prescription AI: Data Sharing Disclosure
Apple App Store & Google Play: Required Disclosure
MedNote uses a third-party AI service to process prescription images. Before any prescription data is sent to this service, you are shown an explicit in-app consent prompt and must actively agree. You can opt out at any time.
This section covers prescription scanning only. Desha also operates an AI voice and conversation service that uses a different set of processors and works differently. It is described separately in Section 6, and you should read that section as well.
5.1 What AI Service We Use
MedNote's Prescription Intelligence Engine is powered by Microsoft Azure AI Services (Azure Cognitive Services / Azure AI Document Intelligence), operated by Microsoft Corporation. Azure AI processes prescription images on our behalf under a Data Processing Agreement that requires equivalent data protection standards to those we apply.
5.2 What Data Is Sent to the AI Service
When you use the Prescription Intelligence feature, the following data is transmitted to Azure AI:
- The prescription image or document you upload (JPEG, PNG, or PDF)
- A unique request identifier (not your name, email, or account ID)
What is NOT sent: Your name, email address, date of birth, account credentials, or any other personally identifiable information is never transmitted to the AI service. The image is processed ephemerally. Azure AI does not retain it after the response is returned.
5.3 What the AI Returns
The AI service returns structured data extracted from the prescription: medication names, dosages, frequency, and prescribing doctor's name (where visible). This extracted data is then stored in your MedNote profile on our secure Azure servers and linked to your account. It is never shared with other users or third parties beyond the hosting described in Section 8.
5.4 Your Consent and How to Opt Out
You are in control. The AI prescription scan is never triggered automatically.
- When you upload a prescription for the first time, MedNote displays a consent prompt explaining that the image will be processed by Azure AI.
- You must tap "I Agree" before the scan begins. Tapping "Cancel" uploads the image without AI processing, and you can manually enter medication details instead.
- You can change your AI processing preference at any time in Settings → Privacy → Prescription AI Scan.
- Withdrawing consent stops future AI processing but does not delete previously extracted data. To remove previously extracted data, use Settings → Account → Delete Health Data.
5.5 Azure AI Data Protection
Microsoft Azure AI Services is bound by the following protections, which meet or exceed our own standards:
- ISO 27001, SOC 2 Type II, and CSA STAR certified
- HIPAA Business Associate Agreement (BAA) eligible
- GDPR compliant. Azure processes data as a Data Processor under our instructions
- Data is not used to train Microsoft's AI models unless you explicitly opt in (we do not enable this)
- Processed data is deleted from Azure's transient cache immediately after response
6. AI Voice Calls, Transcription and Conversation Data
Read this before you take a call from Desha
Desha places and receives voice calls that are conducted by an AI assistant, not by a person. Every call is transcribed word for word and the transcript is stored in your health record. The assistant also keeps a short written summary of the call and a set of facts it extracted from what you said. This section explains exactly what is captured, who processes it, how long we keep it, and how to switch these calls off.
6.1 When the AI Assistant Speaks With You
- Outbound reminder calls: we telephone the mobile number on your profile about a dose that is due or has been missed
- Inbound calls: you call one of our numbers and the assistant answers
- In-app calls: a voice call carried inside the MedNote app over the internet rather than the telephone network. This is the one that uses your microphone permission
- Health Companion chat: typed AI conversations in the app, which are stored the same way as a call transcript
You are always speaking to software. It is not a clinician, and no member of Desha staff listens to your call while it is happening.
6.2 What We Capture and Store
- Your speech, while the call is live: streamed to a speech-to-text service so the assistant can understand you. We do not keep this audio
- A verbatim transcript: every turn of the conversation, yours and the assistant's, with timestamps. Stored against your profile and not truncated
- An AI-written summary of what was discussed and how the call ended
- An audio recording of the assistant's side of a telephone call: a sound file of what the assistant said to you. Your own voice is not stored as audio. In-app calls are not recorded as audio at all
- Extracted memory facts: after the call, an AI model reads the conversation and writes out key facts, such as a symptom, an allergy, a medication you confirmed, or a preferred call time. These are kept against your profile and re-used to give context on later calls
- Call metadata: the number called and called from, date, time, duration, direction, language, the carrier used, and how many turns were exchanged
- An urgency classification derived from the conversation, used to decide whether the call needs to be escalated
Unlike prescription scanning (Section 5), this data is not de-identified before processing. Your name, your phone number and your patient identifier are attached to the stored call record.
6.3 Processors Involved in a Call
Delivering a voice call needs several specialist providers. Each one receives only the part it needs:

Which carrier calls you depends on your country. Numbers in India are called via Plivo. Numbers in the United Kingdom, Germany and France are called via Telnyx. Numbers in the United States, Canada, Australia, Japan, Spain, Mexico, Brazil, the United Arab Emirates and all other countries are called via Twilio.
Working toward
We are working toward executing data processing agreements with each voice processor named above, and toward confirming in writing that none of them retains your call data or uses it to train their own models. Until that is documented we will not claim it. Where a processor publishes its own privacy terms, those terms apply to the leg it handles.
6.4 Automated Processing, and What the Assistant Will Not Do
The assistant can log that you confirmed or missed a dose, answer questions about medication you have already recorded, classify how urgent the conversation sounds, and raise an alert. That is the extent of it.
- It does not diagnose you, prescribe, or change a medication or dose
- It does not make an autonomous clinical decision, and it cannot alter your prescription record on its own
- It is not a medical device, and it is not an emergency service. Nobody is listening live. If you are having a medical emergency, end the call and contact your local emergency services
- We do not use these calls to make decisions about you that produce legal or similarly significant effects by automated means alone
Escalation. If the conversation contains indicators of an emergency or acute distress, an alert may be raised. That alert can include a short excerpt of what you said, and it may be sent to a clinician you have connected to and to the family contacts you have nominated for escalation. Once shared, that excerpt is also held in the recipient's record. See Section 8.
6.5 How to Turn AI Voice Calls Off
AI voice calls are on by default. Unlike the prescription scan in Section 5, they are not gated behind a separate consent prompt. If you have never changed the setting, we treat your account as able to receive them, even if the toggle appears switched off in the app. Setting it explicitly is what records your choice.
- In the MedNote app, go to Settings → Privacy → Voice Call Notifications and switch it off.
- For a family member you manage, use that member's settings in the app, or Family → Member Details on the web.
- You can end any call at any time simply by hanging up.
- To have the calls already stored removed, see Section 6.6 and Section 12.
What changes when you switch it off. We stop placing AI calls to your telephone number. If you have the app installed with notifications enabled, medication reminders continue inside the app, including in-app AI calls. Switching off push notifications as well stops reminder calls on both channels. Withdrawing from voice calls does not delete calls that already happened.
Quiet hours. The quiet-hours setting in the app currently applies to notifications and to family escalation, but not to medication reminder calls, which are placed within the window of the dose they relate to. We are working toward bringing reminder calls under the same quiet-hours control.
We are also working toward a fail-safe default, so that a temporary outage of our preferences service cannot result in a call being placed to someone who has opted out. Today, that failure mode defaults to placing the call.
6.6 Retention and Deletion of Voice Data
We do not currently run an automatic time-based deletion schedule for voice data. Call transcripts, summaries, the assistant-side recordings and extracted memory facts are kept for as long as your account exists, unless you delete them. We are working toward a defined retention period for medical call records in each country we operate in, and we will publish it here once it is set. We would rather state this plainly than quote a figure we do not yet apply.
- The working state of a live conversation is held in a temporary cache that expires 30 minutes after the call ends
- Deleting your account triggers a hard delete of your call records, transcripts, summaries, assistant-side recordings and extracted memory facts, alongside the rest of your data
- Transcripts of in-app calls that appear in your Health Companion chat history are deleted as part of the same account deletion
- Erasure runs across each store independently and on a best-effort basis. If you want written confirmation that every store has been cleared, email privacy@desha.ai and we will verify it and finish the job by hand where needed
- An excerpt already shared with a clinician or family contact through an escalation alert sits in their record and is handled under Section 8
6.7 Gaps We Are Working On
We would rather tell you what is not yet in place than imply it is. As at the date of this policy:
- The assistant does not yet announce at the start of a call that it is an AI and that the call is being transcribed and stored. We are building that spoken notice
- There is no separate consent step for AI voice calls, transcription and recording at sign-up. We are working toward one, with a record of when it was given
- The retention period for voice data is not yet fixed. See Section 6.6
- Data processing terms with the voice processors named in Section 6.3 are being put in place. See Section 6.3
If any of this matters to you now, you can switch AI voice calls off today using Section 6.5, and you can ask us to erase your voice data at any time at privacy@desha.ai.

8. Third-Party Services and Data Sharing
We do not sell your personal or health data. We share data only with the following categories of processors under appropriate data processing agreements:
Microsoft Azure (Cloud Infrastructure)
All application data, health records, and prescription files are stored on Microsoft Azure servers. Azure is certified for healthcare data (ISO 27001, SOC 2, HIPAA BAA eligible). Data is encrypted at rest (AES-256) and in transit (TLS 1.3).

We use Firebase Cloud Messaging for medication and health reminders, and Firebase Analytics for anonymised app usage analytics. Analytics data is aggregated and does not include health records or prescription content.
Firebase Privacy Policy →Deepgram (Speech Recognition)
Converts your speech into text in real time during an AI voice call. It receives the live audio of your side of the call while the call is connected. Processing takes place on Deepgram's own infrastructure, which may be outside your country.
Deepgram Privacy Policy →ElevenLabs (Synthetic Voice)
Generates the assistant's spoken voice in several languages. It receives only the text the assistant is about to say, which can include your first name and the names of your medications. It does not receive your audio.
ElevenLabs Privacy Policy →LiveKit (In-App Call Transport)
Carries the audio of calls made inside the MedNote app. Audio passes through LiveKit while the call is live. We do not store call audio there.
LiveKit Privacy Policy →Twilio, Telnyx and Plivo (Telephone Carriers)
Connect telephone calls to and from your mobile number. Each receives your phone number and carries the call audio in transit. Which one is used depends on the country of your number: Plivo for India, Telnyx for the United Kingdom, Germany and France, and Twilio elsewhere. We do not ask any carrier to record your calls.
Apple / Google (App Distribution)
The MedNote iOS app is distributed through the Apple App Store and the Android app through Google Play. The iOS app does not offer In-App Purchases, so no payment data is exchanged with Apple for subscriptions. App-store crash and device data may be shared with Apple/Google per their standard platform policies.
Healthcare Providers (with your consent)
If you connect MedNote to a DocNote-registered clinician, specific adherence data and health records may be shared with that provider. This can include a short excerpt of what you said on an AI voice call where the conversation triggered an urgent alert (Section 6.4). You control and can revoke this connection at any time from your account settings. Data already shared with a clinician sits in their record and is governed by their own obligations.
Legal and Regulatory Authorities
We may disclose information when required by applicable law, court order, or governmental authority in the EU, India, United States, or other jurisdictions where we operate.
9. Children and Family Accounts
Parental Consent Required
MedNote allows family account holders to add child profiles for dependant health management. Adding a child profile constitutes your explicit consent as a parent or legal guardian to collect and process that child's health information within MedNote.
- The primary account holder must be 18 years or older
- Child profiles are accessible only within the family account. Children do not have independent login access
- Health data for child profiles is subject to the same security and retention policies as adult data
- Parents/guardians may delete a child's profile and all associated data at any time from account settings
- We do not knowingly allow individuals under 18 to create independent MedNote accounts
- If you believe we have inadvertently collected information from a minor without parental consent, contact us at privacy@desha.ai to request deletion

11. Data Security
We apply healthcare-grade security measures to protect your data:
AES-256 Encryption
All data at rest is encrypted using AES-256. All data in transit uses TLS 1.3.
Role-Based Access Control
Strict RBAC ensures only authorised personnel access specific data categories.
Health Data Kept Out of Logs
Health content, including the text of what you say on an AI voice call, is never written into our application logs. It is held only in the health record described in Sections 3 and 6.
DPIA in progress
A Data Protection Impact Assessment covering our high-risk health data processing, including AI voice calls, is in progress and not yet complete.
Self-declared. The measures above are our own controls, assessed by us. Desha AI does not hold a third-party security certification today. Where we describe a certification, it belongs to a named infrastructure provider such as Microsoft Azure, not to Desha AI.
No method of electronic transmission or storage is 100% secure. In the event of a data breach that affects your rights and freedoms, we will notify you and relevant authorities as required by applicable law.
12. Your Privacy Rights
Depending on your country of residence, you have the following rights. To exercise any of these rights, contact us at privacy@desha.ai. We will respond within 30 days.
How to Delete Your Account
- Open MedNote app → go to Settings
- Tap Account → Delete Account
- Confirm deletion. Your data will be permanently erased within 30 days
- Alternatively, email privacy@desha.ai with the subject "Account Deletion Request"
13. Jurisdiction-Specific Rights
European Union & UK (GDPR / UK GDPR)
You have the rights described in Section 12 above under the General Data Protection Regulation. You also have the right to lodge a complaint with your national data protection authority (e.g., ICO in the UK, CNIL in France).
India (Digital Personal Data Protection Act, 2023)
We comply with the DPDP Act 2023. You have the right to access, correct, and erase your personal data. You may nominate another person to exercise your rights on your behalf. Grievances can be raised by emailing privacy@desha.ai.
United States (CCPA / State Laws)
California residents have the right to know what personal information we collect, request deletion, and opt out of any sale of personal information (we do not sell personal data). We honour opt-out requests within 15 business days.
Healthcare Data (HIPAA, US Users)
For users in the United States where HIPAA may apply, MedNote operates as a personal health record application under individual user control. Where we partner with covered healthcare entities, appropriate Business Associate Agreements are in place.
14. International Data Transfers
Your data may be processed in data centres in Europe, Asia, and the United States (via Microsoft Azure infrastructure). Where data is transferred outside your country, we ensure appropriate safeguards are in place including EU Standard Contractual Clauses, adequacy decisions, or other approved transfer mechanisms under applicable data protection laws.
AI voice calls involve additional processors that operate their own infrastructure, which may be located outside your country. These are named in Section 6.3. Your speech may therefore leave your country while a call is being transcribed, and the telephone carrier that connects the call is chosen by the country of your phone number. We are working toward documented transfer safeguards with each of these voice processors, and we will not describe those safeguards as in place until they are.
15. Cookies and Tracking Technologies
Our web platform uses cookies and similar technologies for authentication, security, and analytics. The MedNote mobile app uses Firebase SDK for anonymised analytics and crash reporting.

16. Medical Information Disclaimer
MedNote is a health management tool designed to help you organise and track your medications and health information. It is not a medical device and does not provide medical diagnoses, treatment recommendations, or clinical advice. This applies equally to the AI voice assistant described in Section 6: it is software, it does not diagnose or prescribe, it makes no clinical decision on its own, and no one is listening to your call as it happens. Always consult a qualified healthcare professional for medical decisions. In a medical emergency, end the call and contact your local emergency services immediately.

18. Changes to This Policy
We may update this Privacy Policy to reflect changes in our practices, technology, or applicable laws. When we make material changes, we will notify you via in-app notification and update the "Last updated" date above. For significant changes affecting health data processing, we will seek fresh consent where required by law. Continued use of MedNote after the effective date constitutes acceptance of the updated policy.
